Huge aneurythm on Middle cerebral artery(MCA), which was already
ruptured causing SAH: subarachonoid hemorrhage.

operation: froto-temproral incision. (it is too big for standard
supra-cilliary incision.) open dura. microscopic surgery. fundus of
aneurythm was pinched to make it small, then ruptured. it bleeds
enourmously for 10 secs before clipped. After the aneurythm was
shrinked, the neck was clipped.
